Town of East Hampton – Part Time Community Service Worker – Human Services Department
This is a part-time position, 20 hours per week, year-round.
Under general supervision, an employee in this class performs non-professional duties to assist professional social and community workers in the implementation and delivery of agency programs and services. The incumbent must exercise some independent judgement within well-established guidelines. Work involves considerable telephone or personal contact with clients and the public.
Responsibilities include:
- Conducts in-home visits of the homemakers in each of their clients’ homes twice per year
- Maintain client daily log sheets and enter data in PEER Place, monthly log sheets, homemaker monthly calendars and time sheets to verify the provision of services
- Complete a monthly report for Suffolk County Office for the Aging that describes and updates any program updates or changes that may have occurred during the month
- Generate monthly billing invoices and maintain records for those clients who are required to pay a cost share towards the provision of their EISEP services
- Complete a 599 Voucher to Suffolk County every month so that the own can be reimbursed for services provided
- Monitor the care and maintenance of the four vehicles assigned to EISEP
- Conduct periodic mailings to clients such as client satisfaction surveys, contribution letters, homemaker aide duty letters, summer heat precaution letters and any other material related to the needs of the clients
- Maintain a list of private pay providers and agencies for Town residents who require additional service beyond what we can provide
- Coordinate and update the Special Needs Emergency Evacuation list for the Town of East Hampton
- Assists the Director and Case Managers with reaching out to the community regarding varied programs of senior citizens activities conducted at senior citizens centers and programs
- Provides information and referral, phone calls regarding all Human Services Programs to the community
Minimum qualifications; graduation from a standard senior high school or possession of a high school equivalency diploma, and one (1) year of experience in a position involving substantial client contact, in a public or private social service agency..
